Occupational Health maintains physical records for the Occupational Health Program only. Immunizations records and lab results that took place on/after January 8, 2018 may be accessed through My Health Connection.
Record requests cannot be accepted over the phone or through general email requests. Submit the Medical Records Request electronically to firstname.lastname@example.org. Requests may take up to 10 business days to process after receipt of the completed form. Once located, the requested record(s) will be scanned and sent securely via email to the requester. Electronic delivery is preferred. If a physical copy is required (for pickup by the requester), be sure to indicate this when sending in the completed form. Incomplete or inaccurate information may delay processing.
