Appointments
We welcome your schedule requests via appointment form below. Our clinic days are Tuesday, Wednesday and Thursday. We attempt to schedule appointments as close to preference as possible.
To request an appointment time:
- Complete the Appointment Form at right.
- Submit via email attachment to occupational.health@cuanschutz.edu.
- You will receive a confirmation email once your appointment is made.
