UCHealth COVID-19 and Flu Vaccines

COVID-19 and flu vaccinations may be given the same day in different arms, or may be given on different days with no spacing. However, UCHealth COVID-19 vaccine clinics are not offering flu shots at this time. Drug stores and other vaccine providers may offer both at the same time.

  • Submit your COVID-19  vaccination information to: system.employeehealth@uchealth.org.
    • Include full name
    • Date or birth
    • Employee number
    • Immunization date(s)
    • Vaccine lot number(s)
    • Vaccination manufacturer (e.g. Pfizer)
    • Valid provider signature
