If a patient notifies you of a change in their demographics (e.g. legal name, address, gender identity), the patient can use MyHealthConnection to securely communicate these changes to the appropriate UCHealth staff. If a patient does not have a MyHealthConnection account, they can contact the MyHealthConnection team by phone or email to set up an account. Please refer the patient to our website below.
If the patient prefers, changes to their demographics can be communicated at any visit occurring at a UCHealth facility. In order to protect the patients’ confidentiality and to prevent identity theft, these changes are permitted through the secure MyHealthConnection portal or in person to ensure positive identification that the patient or legal representative is the requestor of the change.
