Subjects' Initials on each page, and investigator signature line removed from template consent forms

We have updated the COMIRB template consent forms to remove lines for subjects' initials from each page and remove lines for an investigator signature.
If your approved consent form includes lines for subjects' initial on each page, or a line for an investigator to sign, please contiunue to obtain these initials/signatures as IRB approved. Submit an amendment if you would like to remove these from your approved consent forms.
